Our Intake Forms

When visiting PawMED, the following form should be filled out before your pet can be evaluated: 

We are NOT an emergency room.

While we are equipped to handle most illness, there are a few things that may require an emergency room, overnight/long term care, or emergency surgery.

If you think your pet is too sick to wait to be seen, we can help direct you and your pet to a 24-hour emergency hospital.

You should always give us a call if you are concerned about your pet and we can help you decide whether PawMed is the right choice.

Payments: We accept most major credit cards, Care Credit, and ScratchPay. We do NOT accept payment plans. Payment is due at the time of service and a deposit may be required.
